"Permanent Patients"

I thought i would share this story about "permanent patients" in the hospital setting. These are patients who are "stuck" in the hospital because they don't have a safe discharge plan. According to data from AHRQ this type of patients are on the rise... at least by 20% since 2005 to 2009. Scary number if you think about it. I'm sure we all have our own stories to add to this.

Understanding ACO

ACO which stands for Accountable Care Organization is a healthcare entity that would be held accountable for improving care while reducing costs through coordinated care. This is all part of Medicare Shared Savings Program. All ACO's that are successfull in providing quality care and at the same time reducing health care costs will share in savings to Medicare. Does anyone know what percentage that is? This fact sheet shows the first 27 ACO's who signed up to be in the program. Two of them are hoping for higher share of savings in exchange for sharing any losses if costs of care increased. Now, that's interesting!
