Co-producing Healthcare with The Veterans

The concept of co-production fascinates me. Studies have shown that empowering patients through co-production yield better outcomes. This article explains how Veterans can manage their own healthcare and be a co-producer of their own health. The VA created a website for them MyHealTheVet.com, where the veteran can manage their own care,schedule their own appointment online, view their lab values and medical records, consult a doctor, fill their prescriptions, etc. I think this is a great way of empowering patients.

Proposed Settlement Agreement That Will Affect Medicare Beneficiaries

Often times, we see patients that are needing further skilled services but because they have reached the plateau, Medicare stops paying. Pretty soon this line " for as long as you show improvement, Medicare will continue to pay for your services" will be changed to "Medicare will pay for you're maintenance therapy". This Proposed Settlement Agreement will hopefully stop CMS from denying patients with chronic conditions the appropriate care they need to continue living a quality life. This will hopefully decrease hospital re-admissions and eventually save us health care dollars.

Medicare's 3-Day Stay Rule Doesn't Make Sense

I am dissappointed to learn that this bill (H.R. 1543 Improving Access to Medicare Coverage Act of 2011) which will eliminate the 3day Inpatient stay rule before SNF admissions only has 1% chance to pass. We need to keep trying because it does not make sense for some patients who does not fall into the 3 day category to waste healthcare dollars.

Medicare to Fine Hospitals For Readmissions

It's coming! Medicare will start to penalize hospitals for readmissions. Penalty will be as low as 1% and is expected to go up to 3%. This is part of the Health Care Reform ...transforming Health Care from fragmented care to continuing care. This will make healthcare providers to be more vigilant in ensuring that patients follow up with their doctors after discharge. What do we do with non-compliant ones? It will be a challenge to drag them to their appointments. I can almost foresee hospitals forming initiatives in attempt to reduce readmissions. This could be a good thing!

I found out today that Sanofi brought back their Lovenox Assistance Program! Good news right? Not so fast...with the program reinstated, they made some major changes. They no longer give you a confirmation letter prior to discharge.They will only deliver to hospital outpatient pharmacy. They actually ask you now for dispense log prior to approval. That means, you would have to risk not getting approved while you wait for reimbursement. Do you know of anyone who took advantage of this program? We want to hear how you did it!

It will hopefully crack down on those "doctor shoppers". It will also promote e-prescribing which will reduce the theft, forgery, or alteration from paper prescription.It will create safe disposal program for prescription drugs. "Partners for Children"- A Beacon of Change for End of Life Care

This article showed significant savings on healthcare costs by eliminating the six month time limit on children with life threatening illnesses before they can enter hospice care. It reduced hospital readmissions by 35 percent , a savings of 11percent per patient. This program "Partners for Children" was piloted in UCLA. I think it's a good program to adapt. It is however hard for many of us to accept end of life. When do you say enough is enough? My opinion is that it's more of the individual's acceptance of end of life that will decide that...it does not matter if the elimination period is there or not. What do you think?

In allignment with our upcoming meeting topic about the impact of obesity, I would like to share this article about a doctor's campaign to fight obesity. I never thought to correlate hairstyle with obesity, but this article explains how a person who spends more on upkeeping their hairstyle could get discouraged to exercise, therefore, lives an unhealthy life. Very interesting correlation and campaign to fight obesity.

Not too long ago I received a prescription discount card in the mail from Nassau County. It has a member number specifically attached to my name and home address. Luckily, I have put it aside despite of my reluctance. It's just too good to be true. Until one of my patient whose medications were not covered by his insurance had a need for it. I almost forgot about this benefit that Nassau County offers for all Nassau resident whose medications are not covered by their insurance. So, I reminded my patient about it and showed them what the card looked like. Unfortunately, she thought it was junk mail and threw it away. The good news is that you can print a copy of the card online. You could save up to 20% off the retail price. I think that's great!

Keep The Noise Down

In our recent collaborative care meeting where I work, we usually score less than what we want on patient satisfaction for keeping the noise level down. There's the beeping of machines, nurses yelling in the hallway, visitors' cell phone ringing, being awakened by a PCA to take Vital Signs in the middle of the night, the lists can go on and on. It's been a challenge to show improvements on these indicators. One nurse suggested each nurse carries a cell phone to limit yelling in the hallway for the nurse. I have to admit, since the hospital installed a new nurse tracking device, I haven't noticed too much yelling in the hallway for their names. Another suggestion was to take less Vital Signs at night only if the doctor medically allows so. This article has more suggestions from other hospitals. Is your hospital ready to go above and beyond like the Department of Veterans Affairs New Jersey Health Care System in reducing the noise level? It's amazing! You have to read it....

Doctors Answers Their Texts Faster Than Pagers

It's the best invention ever! It's not only the doctors but anyone- even my mother with arthritic fingers answers text messages quicker than the phone call. In my opinion, there's a lot more advantages to using this technology as a way to communicate. It improves workflow, increases patients and providers satisfaction, miscommunications with foreign speaking providers will be avoided. (have you ever tried spelling Rolling Walker to a new resident?) It's pretty embarrasing. The problem however, is keeping it secure. This article outlined ways to keep text messages secure. I was reading these new approaches to solving healthcare challenges and what struck me is the part where it mentioned building health resilience technology to withstand natural disasters. It's very important especially that the hurricane season has begun for chronically ill to continue to use their DME's even with prolonged power outages for example. We could save thousands of lives if we have a way of knowing who needs access to DME's at the time of natural disasters.

Breast Density Notification Bill

This is an FYI that in California, Senate Bill 1538 which requires health care providers to notify women of dense breast tissue as detected by mammogram is now headed to the State Assembly and if passed into a law, other states might follow. Accountable Care Initiative in New Hampshire
We can all learn from this new initiative in New Hampshire to improve care coordination and patient outcomes and lower healthcare costs. We, as case managers, continue to be a critical element in these initiatives. Healthcare had come along way, transforming it from provider centered to now patient centered. We also moved from fragmented care to now continuing patient care. In my opinion, today’s providers are more attuned in ensuring that their patients have the right follow up care.

Booming Home Care Agencies

It's good to know that despite our down economy, the home care industry is booming and expected to grow by 1.6 million caregiving jobs by 2020. I wish Suffolk county would have more home care agencies that would widen catchment area to include Fire Island, Amagansett and other Suffolk towns that are hard to reach areas. My colleague just had a hard time looking for an agency in Fire Island the other day. She must have called at least a dozen agencies and none of them can accept. Unfortunately, it isn't the first time.

Elder Care Goes Hi-Tech!

I find it always fascinating to read articles on innovation involving healthcare and our elderly population. This article provides different kinds of technology that is out there and is being tested to be effective. I love the GlowCap idea (sends alert to family/doctors everytime it's opened), what a way to monitor patient compliance. I also liked SimpleC ( a device to help improve memory for patient's with memory loss). I think we are going to see a lot more innovations as this generation embraces the changes that are evolving. I hope you enjoyed the article as much as I did.

"Permanent Patients"

I thought i would share this story about "permanent patients" in the hospital setting. These are patients who are "stuck" in the hospital because they don't have a safe discharge plan. According to data from AHRQ this type of patients are on the rise... at least by 20% since 2005 to 2009. Scary number if you think about it. I'm sure we all have our own stories to add to this.

According to data from AHRQ this type of patients are on the rise... at least by 20% since 2005 to 2009. Scary number if you think about it. I'm sure we all have our own stories to add to this.

Understanding ACO

ACO which stands for Accountable Care Organization is a healthcare entity that would be held accountable for improving care while reducing costs through coordinated care. This is all part of Medicare Shared Savings Program. All ACO's that are successfull in providing quality care and at the same time reducing health care costs will share in savings to Medicare. Does anyone know what percentage that is? This fact sheet shows the first 27 ACO's who signed up to be in the program. Two of them are hoping for higher share of savings in exchange for sharing any losses if costs of care increased. Now, that's interesting!

Medicaid Expansion Might be a Good Thing

I think Medicaid expansion is a good thing because more uninsured Americans will now qualify for Medicaid in hope to fill in the gaps in coverage. However, I agree that this might make it difficult for current medicaid insured because increase in patients could mean longer wait time for appointments. I like the fact that the Affordable Care Act is looking into provisions to attract physicians serving Medicaid population. It's a give and take situation. Do you agree, however, that the provision of Affordable Care Act is unconstitutional?

Alzheimer's Disease - Fatal and Costly

Very scary but it is the reality. Alzheimer's is not only fatal it is also very costly because it is a slow progressing disease.They also attribute it to the fact that 1 of 7 Americans live alone, therefore, they are isolated and changes may not be as noticeable. I can imagine why it will costs $200 billion in 2012 to care for a patient with Alzheimer's. Some article I read even said that the cost will rise to 1.1 trillion in 2050 which then makes Alzheimer's a public health emergency. I think we need to invest in research to find cure, invest in early detection programs or public education on how to care for those who has Alzheimer's. It might help bring the costs down.

Yoga and Medicare

Not too long ago, I posted a topic on Alternative Medicine and payment options for such, now hearing about Medicare paying for programs that includes Yoga classes as a mean to reverse heart disease is great news and only proves that Health Care is making great progress in recognizing ways to reduce healthcare costs. Does anyone heard of anybody taking advantage of this? Do we know of any program in New York similar to this? If you know of any, please share your story.

This "You Gave, Now Save" Campaign was recently launched by National Association of Area Agencies on Aging in partnership with National Council On Aging to make low-income seniors and their families aware of private and public agencies that offers assistance with their health, food, prescriptions, heating, housing, transportation and many more that a lot of low-income seniors might not even realize they qualify for.With the approaching tax return filing day, it's good to know that we could refer these low- income seniors to TCE (Tax Counseling for the Elderly) program which provides free tax counseling and basic return preparation for those who qualify. To find out more about this campaign download a copy of the pamphlet.

Fair Labor Standards Act Should Include Home Care Aides

It looks like the present Administration is trying to do the right thing by proposing regulations that would provide for minimum wage and overtime pay for home care aides. Majority of the home care attendants I have had a chance to work with are hard-workers and well deserving. So, if you want your opinions to be heard, you have up to February 27th to weigh in on this issue. See article where to send in your public comment. Let's help this proposed policy pass.

Interesting technology. It would be amazing if a diabetic can check his/her blood sugar through an iphone attachment which will then send an alert to his/her doctor using cloud technology- thus avoiding a potential ER visit. What a great idea! I'm all for it. I bet it will be costly. This could be a potential Health Care Policy.What do you think?

This is a sad news to those needing lovenox assistance. I have utilized Sanofi's assistance program several times in the past and was really satisfied with their efficiency. I guess this is part of cutting back on expenses. Does anyone know of any other options besides the one mentioned in the article?

This is a very interesting article. I highly recommend reading it. It is very informative and has CE as added bonus! It makes me asks how many of you are using a predictive model in your organization to avoid re-admissions? The article also explains the changes in thinking that are needed to adapt the Accountable Care Organizations Models of Care. I firmly believe in developing patient-family-physician-nurse relationships in making discharge decisions. I also think patients are ready for discharge when they “feel and think” they are ready. This article certainly elicits my curiosity in developing predictive models to prevent re- admissions.

New Year's Resolution
Hello everyone! Since it is early January, I know a lot of you are probably writing down lists of resolutions. I am guessing that a lot of you would like to manage your stress level, I know I do. So I’d like to share this article on Quick Stress Relief. My favorite one is “taking a break from technology”. There are a lot of electronic stimulations present now than ever before, that I think it is important to unplug. We need to surround ourselves with positive energy. It will be an awesome 2012!
